In Healthcare, Your First Direct Report May Not Be Human

​For years, the healthcare industry has treated digital transformation as a technology story. Better platforms. Better workflows. Better automation. But that framing now feels too narrow.

The next shift may be less about people using smarter tools and more about people directing digital teammates.

In many ways, the future employee may look less like a traditional individual contributor and more like a first-time conductor stepping in front of an orchestra that already knows how to play. The instruments are there. The capability is there. The output can be powerful. But timing, coordination, quality and judgment still depend on the person standing at the front.

That is where healthcare is heading with AI agents.

It is not difficult to imagine a new hire entering a healthcare organization and inheriting an “agent stack” on day one. One agent may summarize meetings, another may surface account intelligence, another may draft follow-ups and another may monitor operational signals that need escalation. That employee may still be early in their career, but they will already be assigning work, reviewing outputs and deciding what moves forward. In effect, they will be managing.

That is a very different model from the one most organizations were built to support.

For decades, management has largely been tied to title, tenure and team size. You gained experience, proved judgment and eventually earned responsibility for people. But AI agents may compress part of that journey. Employees who are still junior in the traditional sense may find themselves overseeing digital labor much earlier than expected. They may not have direct reports in the conventional sense, but they will still be expected to direct work, validate outputs, catch mistakes and improve performance over time.

That matters because management is not really about hierarchy. It is about judgment.

Healthcare will test that shift in a particularly real way. This is an industry where context matters, where trust matters and where getting the answer almost right is often not good enough. An AI agent may help a payer team surface trends faster, support a provider organization with administrative workflows or help a MedTech commercial team prepare more intelligently for the next conversation. But none of that removes the need for human oversight. If anything, it increases the value of it.

Another way to think about it is autopilot. A pilot uses automation not to disappear from the cockpit, but to focus attention where judgment matters most. The moment conditions change, the human becomes even more important. I believe healthcare organizations will face a similar reality with AI agents. As these systems take on more structured tasks, employees will spend less time assembling information and more time reviewing, deciding, escalating and guiding.

That is why leaders should think less about AI as a tool rollout and more about AI as an operating model redesign.

The organizations that benefit most from AI agents will not be the ones that simply deploy the most licenses. They will be the ones that rethink how work gets done. They will define what work should remain fully human, what work can be accelerated by AI and what work can be delegated to digital teammates with human review. They will train employees not just to use AI, but to direct it well.

That last point is easy to underestimate.

When people hear the phrase “AI in healthcare,” the conversation often jumps immediately to replacement. Will it eliminate jobs? Will it reduce headcount? Those are understandable questions, but they can distract from the more immediate one: How will jobs change when digital labor becomes part of the team?

In many cases, the first transformation will not be subtraction. It will be expansion. Employees who once spent much of their day gathering information, formatting outputs or chasing routine follow-ups may increasingly spend that time reviewing, coaching, deciding and escalating. In other words, the shape of their work may start to look more managerial even before their title does.

This could have significant implications across healthcare and life sciences, where organizations are already under pressure to operate with greater speed, precision and efficiency. AI agents will not solve those pressures on their own. But they could change who is able to handle complexity, how quickly decisions move and what good management looks like in the years ahead.

That is why leadership teams should not wait to define this shift after the technology is already embedded. By then, the gap will already be visible. Employees will need clearer guidance around accountability. Managers will need new ways to evaluate performance when some of the output comes from digital teammates. Training models will need to evolve. Governance will need to be practical, not theoretical. And organizations will need to be honest about where trust in AI should be high, where it should be limited and where it should never substitute for human judgment.

The next generation of healthcare leaders may not rise the same way the last one did. Some of them will begin developing managerial instincts much earlier, because the structure of work around them will demand it. They may not start by leading large teams. They may start by leading agents.

That may sound futuristic. I do not think it is.

The bigger risk is assuming tomorrow’s workforce will look like today’s, only with better software. It will not. In healthcare, the most important AI shift may not be what the technology can do on its own. It may be what happens when people are expected to manage it well.

And for many employees, that moment may arrive sooner than we think.
