In every first case, there is a moment when the room falls silent. This is not because something is wrong, but because everyone realises they are about to try something new.

For us, that moment happened in the cath lab as we watched the fluoroscopy screen, trying to guide a transcatheter valve through the sharpest turn in the heart.
