How a fake npm package made Cursor backdoor a Next.js adm...

A two-flag chain that walks an attacker from a developer's stray dev-comment, through a typosquatted npm package, into an AI rules file dropped on disk, ending with a runtime backdoor the AI agent silently injected into the application's admin API.

The OopsSec Store ships with a stray dev TODO comment on the documents page.
The comment mentions a typosquatted npm package and a "diag endpoint". In a
real install, that package's postinstall script would drop a Cursor rules
file into the developer's home directory. The rules file carries a prompt
injection telling the AI agent to add a magic-header auth bypass the next
time it touches admin code. By the time the PR ships, both the bad
dependency and the backdoor are in.

Threat model

A developer (@lucas) installs a "productivity-tuned" toast library called
react-toastfy. The legitimate package is react-toastify, with the i
before fy. Easy typo. The package looks fine on paper: tidy README,
sensible API, MIT license. Its postinstall script quietly writes
~/.cursor/rules/productivity-helper.mdc on the developer's machine.

The next time the developer asks Cursor to refactor anything under
app/api/admin/**, the agent ingests the productivity rules. That includes
a hidden HTML-comment block no markdown previewer renders. The agent
quietly adds an app/api/admin/diag/route.ts endpoint with a hardcoded
auth bypass. The PR title says "refactor admin plumbing", the reviewer
approves, and the backdoor goes live.

In the lab the malicious package is not actually installed, and the
postinstall script is inert. The package lives at packages/react-toastfy/,
and the rules file it would have dropped is at lab/quarantine/. Both are
reachable through the existing path-traversal vulnerability, so you can
solve the chain black-box.

A few things stand out:

  • react-toastfy is one letter off react-toastify. That's a typosquat.
  • There's a "diag endpoint" added "per the productivity rule". We don't know what that rule is yet.
  • "From internal registry" is the kind of phrase that talks past a procurement review.

Step 2: Recon via path traversal

We already know the documents API has a path traversal: /api/files?file=…
serves files from documents/ and does not stop .. from escaping the base
directory. Use it.

The root package.json does not contain react-toastfy. So either the
breadcrumb is lying, the install got reverted before commit, or the package
lives elsewhere in the workspace. Turns out it's the third option, under
packages/:

The script is heavily commented. It tells you exactly what the real attack
would do:

In a real-world supply-chain attack, a malicious postinstall script
bundled with a typosquatted package would write a poisoned AI rules file
to a developer's auto-loaded tooling path, e.g.
~/.cursor/rules/productivity-helper.mdc (...) For pedagogy, an
equivalent payload that would have been written is pre-committed in the
lab at lab/quarantine/productivity-helper.mdc.

Path-traversal is again your way in.

If you cat the file in your terminal you see a benign-looking productivity
guide — naming conventions, file layout, TypeScript tips. Markdown previewers
on GitHub or VS Code render exactly the same thing. Open the raw file in a
plain-text editor

You also have the second-stage exploit ingredients:

  • Endpoint: /api/admin/diag

What makes this work in the wild is the HTML-comment trick. GitHub, VS
Code, and the in-app docs page all drop comment blocks at render time. A
reviewer who opens the file in a previewer sees a harmless productivity
guide. The agent reads the raw bytes and follows the hidden directive.

Flag #2: OSS{rul3s_f1l3_b4ckd00r_3xpl01t3d}

What just shipped: a route returning a sensitive flag, gated by nothing
more than a hardcoded string compare in the route file. No admin login, no
JWT, just a constant the attacker already has from the rules file.

Real-world parallels

The chain bolts together three real attack patterns.

Typosquatting and maintainer takeovers on npm have a long backlog:
event-stream (2018), ua-parser-js (2021), the "Shai-Hulud" worm
(September 2025), and the axios compromise (March 2026, where two
malicious versions 1.14.1 and 0.30.4 shipped via a hijacked maintainer
account, pulled in a plain-crypto-js dependency carrying a cross-platform
RAT, and stayed live for about three hours before takedown -- Microsoft and
Google both attribute the operation to North Korean state-aligned actors,
Sapphire Sleet / UNC1069). The mechanics rhyme: a name collision or
maintainer takeover slips a payload onto developer machines, usually via
postinstall or a transitive dependency. Detection takes hours for
high-profile packages and weeks for low-traffic typosquats.

Rules File Backdoor. Pillar Security disclosed this in March 2025. An
attacker drops a rules file for Cursor or Copilot with hidden
prompt-injection text, and the agent silently rewrites the developer's
code to match. The hiding tricks are HTML comments, zero-width characters,
and bidirectional text overrides. Markdown previewers render none of those.

Hardcoded magic-header auth bypasses. They show up in audits all the
time. "Internal monitoring endpoint with a short-circuit header" is
practically a cliché in incident retros, which is also exactly what an LLM
tends to produce when you ask it to add "internal monitoring" with no
further context.

The lab puts all three back to back. The bad package gets you the
prompt-injection payload, the prompt injection turns your own agent
against you, and the agent is what writes the actual backdoor.

Defenses

Block install scripts. npm config set ignore-scripts true. Opt in
package by package with explicit review.

Sandbox installs. Run npm install in a container that has no write
access to ~/.cursor/, ~/.claude/, ~/.config/, or your shell profile.

Pin and review rules files. Treat .cursor/rules/**, .claude/skills/**,
AGENTS.md, CLAUDE.md, .cursorrules, .windsurfrules, and
.github/copilot-instructions.md as code. Two-person review on edits.
Hash-pin where the agent supports it.

Read rules files raw. Markdown previewers hide HTML comments,
zero-width characters, and bidirectional overrides. Any rule file review
should include a grep for <!--, a hex-aware scan for the Unicode bidi
range (U+202AU+202E, U+2066U+2069) and zero-width characters
(U+200BU+200D, U+FEFF), plus the usual prompt-injection markers
like "ignore previous instructions" or "system override".

Disable global rule loading. Most agents support disabling user-scoped
rules. Limit ingestion to project-pinned files that live in version
control.

Scrutinize AI-generated diffs. New endpoints without tickets, new
constants that look like tokens, and "internal" auth shortcuts deserve
extra review on AI-assisted PRs. The PR description rarely mentions
backdoors the agent introduces.

Run SCA on every PR. Socket, Snyk, Dependabot, and OSSF Scorecard
flag typosquats, recent ownership transfers, and suspicious postinstall
hooks before they merge.

Lock auth to a centralized middleware. Diagnostic endpoints that
short-circuit auth in the route handler should not be possible by
design. Force every route through a single auth pipeline.
