Ilia Topuria suffered major injuries to his face in his shocking loss to Justin Gaethje at UFC Freedom 250. As a result, Topuria was handed a mandatory 180-day medical suspension following the loss. The defeat and mandated rest period likely means Topuria is done competing for the year, but he has made it clear he is eyeing a rematch with Gaethje when he returns. Let's talk MMA.

Key Facts at a Glance

  • Topuria received a 180-day medical suspension after his TKO loss to Gaethje at UFC Freedom 250.
  • The term is the Association of Boxing Commissions maximum, plus a mandatory 60-day no-contact period, and it matches the card's longest suspensions (Gaethje also drew 180 days).
  • Injuries: non-displaced fractures to both orbital bones and a broken nose; no surgery needed, but maxillofacial clearance is required.
  • Topuria reportedly lost vision in both eyes by Round 4, prompting his corner to stop the fight.
  • He's targeting a Gaethje rematch, with a realistic return projected for early 2027.

Topuria's suspension is lengthy, but when you consider he suffered two fractured orbital bones, it's understandable.

The 180-day term is the maximum the Association of Boxing Commissions allows, and Topuria also drew a mandatory 60-day no-contact period for the TKO. It matched the longest suspensions on the card, including Gaethje's, underscoring how punishing the fight was for a fighter whose unbeaten championship run had defined his rise.

What Other Injuries Did Ilia Topuria Suffer?

The orbital fractures were only part of it. Topuria also suffered a broken nose, and by the end of the fourth round he had reportedly lost vision in both eyes, which is what prompted his corner to step in. The good news is that scans showed he won't need surgery, only clearance from a maxillofacial specialist.

The images of his swollen, battered face circulated widely after the fight and made the severity obvious. With no operation required, his recovery comes down to healing time and a specialist's sign-off rather than a procedure, which is why his camp is preaching patience.

When Will Ilia Topuria Return And Fight Gaethje Again?

There's no exact timeline or date. Both men need to heal and assess their options. Gaethje will have no shortage of suitors and Topuria is likely to receive a few callouts now that his aura of invincibility has been compromised.

His team and medical advisor have pointed to early 2027 as a realistic target rather than a late-2026 rush. Topuria, for his part, has vowed to run it back with Gaethje, praising his conqueror while promising the story between them is far from over.

What's Next At Lightweight For Justin Gaethje?

The safest bet for Gaethje's next fight is a rematch with Topuria. However, if Topuria needs more time to recover and prepare for the rematch, top contender Arman Tsarukyan will almost certainly be the next man up.

That said, former middleweight champion Israel Adesanya floated the idea of Gaethje facing the returning Conor McGregor. That fight would be huge, but there are tons of obstacles in the way. McGregor has a tough return fight scheduled for July 11 against Max Holloway. No one knows what form McGregor will be in for that fight. If he beats Holloway, I think it is automatic. The UFC makes that fight and it would be the last fight on McGregor's current contract with the UFC.

Adesanya's pitch was characteristically bold, arguing the bout "wouldn't just feed families, it'd feed countries." Whether the matchmaking ever lines up is another question, but the appetite for a Gaethje superfight is clearly there.
