You Don't Have A QA Problem; You Have A Trust Problem

​I have spent 16 years working with enterprise engineering teams. The pattern I see most often is not a shortage of talent or tools. It is a shortage of confidence. Teams that can build quickly still release slowly, not because the code is not ready but because nobody in the chain is willing to say "ship it" without one more review, one more sanity check or one more week of buffer.

That gap between what an organization can build and what it actually ships is not a performance issue. It is a confidence deficit. And in my experience, it is one of the most expensive problems nobody names correctly.

The real cost isn't bugs. It's hesitation.

Here is what I see repeatedly: An engineer finishes a feature. It works. But they split the pull request into three smaller ones, thinking it's safer that way. The lead asks for manual retesting because the last deployment broke something. Product pushes the launch back a week "just to be safe."

Now multiply that across an organization:

• Fifty developers hedging on every commit

• Ten squads padding their sprint estimates

• A quarterly roadmap that drifts by a month every cycle

• An exec team asking "Are we sure?" before every release

None of those decisions are irrational. The last time something shipped without extra scrutiny, it caused an incident.

CISQ's 2022 report found that poor software quality cost the U.S. economy $2.41 trillion. Of that, $1.52 trillion came from technical debt and rework alone.

The fear comes from what teams can't see.

What drives the hesitation is rarely bad code. It is the absence of a clear line of sight into what happens when code hits production. For example:

• No reliable signal on which environments will break

• No confidence that the regression suite covers real user paths

• No visibility into the blast radius of a failed deployment

When engineers cannot see what is on the other side of the merge button, they slow down. That is not a discipline problem. It is a visibility gap.

I watch CTOs respond by looking at the pipeline.

They tune the CI/CD. They audit the SDLC. They add automation. Those things help at the margins. But they miss the root cause. The organization has no reliable signal for "this deployment is safe." So, every person in the chain adds their own safety margin. Those margins stack. A two-week sprint becomes a five-week go-live.

Shrinking the gate doesn't fix it, either.

Most engineering organizations still treat validation as a sequential gate. Code gets written, checked and shipped. That worked when launches happened monthly. At modern cadences, it creates a bottleneck by design.

The 2024 DORA "Accelerate State of DevOps" report shows how wide the gap is. Elite teams deploy multiple times per day, recover from failures in under an hour and maintain change failure rates as low as 5%.

Meanwhile, the low-performing tier takes months to deploy and weeks to recover. That tier grew to 25% of surveyed teams in 2024. The difference is not talent. It is infrastructure.

When CTOs spot this problem, the response I see most often is to shrink the gate:

• Cut coverage scope.

• Skip edge cases.

• Move to a "test in production" posture.

The fear does not go away. It changes shape. Instead of cautious deployments, you get longer incident response cycles. Instead of hedging before the push, engineers hedge after it.

The financial exposure gets worse. Ponemon Institute estimates the average cost of IT downtime at $9,000 per minute.

"Test in production" is a bet that your organization can absorb those numbers when the bet goes wrong.

What actually fixes this?

The CTOs I have seen break out of this cycle all do one thing differently. They stop treating validation as a gate that slows things down. They treat it as infrastructure that speeds things up.

Think about observability. Nobody debates whether to invest in monitoring with AI-powered tools. It is table stakes. A mature quality engineering function deserves the same treatment. When an organization has repeatable, high-coverage verification that runs at pipeline speed and produces results developers trust, people stop hedging, pull requests move faster and launch scope gets bigger.

That does not happen because someone mandated velocity. It happens because the fear that suppressed it went away.

This is the inversion most CTOs miss. The conversation starts with "our release validation is slowing us down."

From what I have seen, it is the absence of a trusted validation program that slows things down. When verification is unreliable, everyone builds workarounds. Extra reviews. Manual smoke tests. Smaller deployment windows.

Strip those away, and what you have is not a heavy process. It is a confidence gap filled with human overhead.

What is the dividing line?

After 16 years in this space, I can say this with confidence: The dividing line between CTOs who ship predictably and those who don’t rarely come down to developers.

It comes down to one question: Is your quality function treated as overhead or as infrastructure?

Overhead gets cut when budgets tighten. Coverage gaps widen. Confidence erodes. Infrastructure gets funded, protected and built to scale.

NIST's research established the baseline: The cost to fix a defect multiplies at every stage of development. That multiplier shows up as support tickets, rollback cycles and customer trust erosion. In regulated sectors, it becomes compliance exposure.

The organizations I have worked with treat validation as an infrastructure ship on rhythm. Their incident rates are lower, not because of a magic process but because their developers operate with certainty instead of caution.

That certainty compounds across hundreds of merges and dozens of launches. It is what separates a two-week cadence from a six-week one.

If your release cadence does not reflect the talent you hired, the problem is probably not your engineers. It is the signal they are missing.​
